Looking For A Template

I'm trying to find a template of a top folding card that I can put my photo on the front to display on the web. Does anyone have one? Or, even tell me how to make one in Photoshop 7? Thanks a bunch!

When you say "top folding card" do you want one that is animated and the top part opens, or are you looking for a flat template that looks like a partially opened card or ??

I'm sorry! I probably didn't make it very clear. No, not animated. I'm looking for it to be flat but so that you can see that it is an open top folding card. But I don't know how to get my photo onto it either in that direction. Thank You!
